Role of Clinical Pharmacists in Optimizing Treatment Decision-Making: A Review

Abstract

Clinical pharmacists are medication-use experts who contribute substantially to evidence-based, safe, effective, and patient-centered treatment decision-making. Their role has expanded from traditional dispensing activities to direct participation in medication selection, therapeutic optimization, monitoring, patient education, medication reconciliation, and collaborative management of disease states. Clinical pharmacists evaluate patient-specific factors such as diagnosis, comorbidities, allergies, renal and hepatic function, laboratory findings, medication history, adherence, drug interactions, and potential adverse drug reactions before recommending pharmacotherapy. Their participation in multidisciplinary healthcare teams can improve medication appropriateness, reduce medication-related problems, and support better clinical outcomes. Pharmacists may also contribute to treatment decisions through medication therapy management, collaborative drug therapy management, antimicrobial stewardship, therapeutic drug monitoring, and pharmacokinetic dose adjustment. Evidence from systematic reviews suggests that clinical pharmacist involvement can reduce adverse drug events and medication errors while improving medication adherence, knowledge, and appropriateness of therapy. Pharmacist participation in post-discharge medication review and collaborative transitional-care services may also contribute to improved clinical outcomes and reduced hospital readmissions, although the magnitude of benefit varies among interventions and patient populations. In chronic disease management, pharmacist-led medication reviews have demonstrated improvements in selected clinical parameters, including blood pressure, glycemic control, and lipid concentrations. Effective treatment decision-making requires collaboration between pharmacists, physicians, nurses, patients, and other healthcare professionals. Clinical pharmacists therefore represent an important component of modern patient-centered care by integrating pharmacological knowledge with individual patient characteristics and clinical evidence. Their expanding involvement in treatment decisions can enhance medication safety, therapeutic effectiveness, adherence, continuity of care, and overall quality of healthcare.
