Drug Utilization Evaluation: An Essential Tool for Optimizing Medication Use and Improving Patient Outcomes

Drug Utilization Evaluation (DUE) is a systematic, criteria-based approach used to assess and improve the appropriate, safe, effective, and economical use of medicines in healthcare settings. Rational medication use requires that patients receive appropriate medicines according to their clinical needs, in suitable doses, for an adequate duration, and at an affordable cost. DUE helps identify inappropriate prescribing, dispensing, administration, monitoring, and continuation of pharmacotherapy by comparing actual medication-use practices with predefined evidence-based standards. It may be conducted prospectively before medication dispensing, concurrently during treatment, or retrospectively after therapy has been completed. Important components include selection of medicines or therapeutic areas for evaluation, development of explicit criteria, collection and analysis of utilization data, identification of deviations, implementation of corrective interventions, and subsequent reassessment. Standardized approaches such as the Anatomical Therapeutic Chemical/Defined Daily Dose (ATC/DDD) system and World Health Organization prescribing indicators facilitate measurement and comparison of medication utilization. DUE has particular importance in antimicrobial stewardship, polypharmacy management, high-risk medication monitoring, medication safety, and cost-effective pharmacotherapy. Evidence from drug utilization studies has demonstrated continuing problems such as inappropriate antibiotic prescribing, polypharmacy, incomplete prescriptions, and inadequate generic prescribing in several healthcare settings. Effective DUE requires collaboration among physicians, pharmacists, nurses, clinical pharmacologists, microbiologists, epidemiologists, and healthcare administrators. Integration of DUE with clinical guidelines, pharmacovigilance, medication reconciliation, antimicrobial stewardship, and electronic clinical decision-support systems can further improve medication-use quality. Ultimately, DUE should function as a continuous quality-improvement process focused not merely on reducing medication utilization or expenditure, but on improving therapeutic effectiveness, patient safety, clinical outcomes, and responsible healthcare-resource utilization.
